When injuries occur at work, there usually can be lasting consequences for both the employee and employer.

If you are managing the case of an injured worker who requires Interventional Pain Management services or Care for their neck or back injuries, we invite you to call Spine Institute of Central Florida. We will be happy to speak with you about any questions you may have regarding the employee and their Workers’ Compensation injury, or any questions relating to an upcoming or previous appointment.

To request an appointment for the injured worker or your client, please download, print, complete and then fax the Workers’ Compensation Appointment Request Form to 863-688-4430.

We guarantee same day appointment for new injured workers evaluation or at most the next clinic day appointment.

Please never hesitate to call and ask to speak with Evan Marvin if you have any specific concern(s) or if you have any suggestions you want to bring to our attention to help your injured worker(s) or the services we provide to them. Our goal is to always maintain a very good communication with you so that you are continually in the loop as to what is being done.

We will always promptly return your calls and complete any requested updates on paperwork documentations. Additionally, we always fax over the completed DFS-F5-DWC-25 Form on the same day in which your injured worker was evaluated or treated.

We get your injured worker safely back to work with any medically appropriate functional limitations and restrictions specified. Our goal is to safely return your patient to their pre-injury status.
